( HROW ), a small biotech company focused on acquiring and commercializing ophthalmic pharmaceutical products in the U.S., dates from October of last year. With the price around $39 and a fair value estimate of $72.7, I saw it as a "Buy". Adding high revenue growth estimates of around 40% but acknowledging a very high uncertainty due to most of their cash flows being far into the future, I was long about half a position. After a gradual increase, the stock dropped like a rock after the last earnings call , when they came with a more modest outlook for 2026. However, I argue that this short-term picture has just a minor importance; the mid-to-long-term horizon is much more important. I will update below my mid-to-long-term estimates, my fair value estimate, and my position, where I see a combination of shares and a bullish short put position (partially covered) as much more interesting than a simple stock position. Business Highlights I start from an updated look at their products, latest developments, and peak sales estimates: Harrow 2025 Revenues (Earnings Presentation) Vevye (for DED, or dry eye disease): $88.7M revenue in 2025 and 14% YoY growth in Q4, driven by their VAFA (Vevye Access For All) program. However, they won the "Preferred Status" with CVS Caremark from January 2026, which will likely help Harrow to get better coverage. From ~9M DED patients treated with an Rx medication, only ~1.5M are managed long-term because of side effects, slow onset of action, or convenience (many prefer OTC drops for immediate but temporary relief, or they use drops only when they feel symptoms). However, Vevye came with remarkable refill rates of 9X in 2025, proving its superiority. For 2035, I think that an average of 500K patients x 6 refills = 3M doses is achievable.
